Attention Licensees:
This year’s licensing
renewal process
began on April 1 and
will run through June 30.
You
should
have received
a postcard in the mail with
renewal information,
including
your Renewal I.D.
Please retain your postcard
as you will need your
Renewal I.D. in order to
renew your license online.
The Board charges a $15
administrative processing
fee for online renewals and
a $50 administrative
processing fee for renewals
by paper application.
If you received a paper
renewal in the mail instead
of a postcard, you will not
be able to renew online and
the administrative
processing fee will be
waived.
If you haven’t received
either a postcard or a paper
renewal, please contact the
Board office.
Please note:
Submission of in-office
surgery reporting forms for
2011-2012 is required for
all licensed
medical doctors, whether
in state, out of state,
active or inactive status.
You must complete and submit
the requisite in-office
surgery form to the Board
prior to
renewing your license, as
you will be required to
attest on your renewal
application that you have
done so. Your license
cannot be renewed without an
affirmative attestation on
the renewal form, so please
do not begin the renewal
process until you have
submitted your in-office
surgery form. Following is
